ID cards
In cooperation with the Polish Consulate, we provide student ID cards to our students. The Consular Section of the Polish Embassy in London issues and stamps student ID cards for our students.
A school ID card is valid in Poland only together with an ID document with a photo (passport or ID card).
Instructions on how to complete the application for a school ID.
The application should be:
fill in in capital letters, e.g. ID CARD
we enter one name and surname, e.g. JAN KOWALSKI
date of birth
highlight whether it concerns the issuance or extension of an ID card:
Issue – please submit the completed application to the school
Extension – application + ID card from last year
ID cards are issued to students who are 6 years old or older.
The completed application should be returned to the class teacher at the beginning of the school year.
If the ID card is lost or damaged, it is possible to apply for a duplicate for a fee.
